KRHIT helps patients in Knox, Laurel and Whitley Counties get the right care at the right place at the right time.
Patients often have barriers that prevent them from receiving the full benefit of the healthcare system. Barriers can include; lack of necessities (food, housing, utilities, etc.), transportation needs and inability to afford medications. P.A.C.T. is a program that links patients with services at no charge to patients. While we target patients diagnosed with congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and Pneumonia, we will assist with other diagnoses as well.
With the current healthcare environment shifting from fee-for-service to value-based purchasing, P.A.C.T. can help improve patient outcomes. We provide in-home assessments, medication reconciliation, plan symptom management and arrange transportation, among many other interventions. We maintain contact with patients’ healthcare providers while they are in the program to provide a team approach to meet their needs.
It can be overwhelming trying to meet the needs of those we care for. There can either seem to be no help available or too much information to process. P.A.C.T. can help by providing you with a Patient Navigator, who is a Master’s Prepared Social Worker. She has over 25 years of experience in our communities, giving her advanced knowledge about available resources. She will work one-on-one with the patient and/or family to link you with resources that are available meet your needs.
There’s an old saying “it takes a village to raise a child.” The same can be said about meeting the needs of our community members. In Knox, Laurel and Whitley counties, we have many obstacles to overcome, including poverty and poor health outcomes. These obstacles are too large for any one organization to conquer. At P.A.C.T., we partner with organizations within our communities to broaden the reach of our services and to better meet the needs of our patients.
